Health Behaviour Modification

Overview:

For this assignment, you will assess the health status of an individual you wish help with a health issue. You will select one, significant health-compromising behavior that you desire to reduce/eliminate or one health-promoting behavior that you desire to add/increase in frequency with your client. The behavior you select will serve as the basis for your behavior modification project. The behavior modification project will run for six weeks. Please incorporate the theories you learned in class into your journal. You will complete a weekly journal of your client�s progress in relation to their health goal. Work on only one health behavior per week, be specific. After writing your introduction, I strongly recommend using the Transtheoretical Model (TTM), the first week you work with your client and assessing what stage he or she is at.
