In the word document, in the column ‘Initial Review Comments’, states what needs to be fixed in my IRB application (see the pdf). Please fix the information in the IRB application (see the pdf) based on the comments in the word document. I also attached the proposal (doc) so that you may have a complete understanding of the information. If possible highlight the changes or state what the changes were. Please return the IRB application in pdf form.
