Nursing- VipulPatel

VipulPatel is a 43 year old gentleman who works in sales and marketing. Vipul lives with his wife Gita and 11 year old daughter Ligi in a three bedroomed terraced house in Bermondsey. Gita works part time in her local playgroup 2 days a week.

Vipul is a social drinker and smokes approximately 30-40 cigarettes a day. Vipulis 177.5 cm tall, weighs 135.8 kg and is classified as obese with a BMI of 43.1. Vipul does not exercise regularly and as he often works late, he tends to sit at home in front of the television with a takeaway or ready meal when he gets home. Vipul’s blood cholesterol level is high at 249mg/dl and he suffers shortness of breath on exertion.

Whilst out having dinner with his family at the weekend, Vipul began feeling unwell and complained of shortness of breath and crushing pain in his chest. Shortly after Vipul collapsed with a suspected heart attack and an ambulance was called.

On admission to A&E Vipul was unresponsive and required chest compressions and defibrillation before being stabilised. Blood tests, an electrocardiograph and echocardiogram revealed Vipul had suffered a myocardial infarction. 

Vipul is now stable and has been commenced on medication to maintain cardiac function whilst he awaits a coronary artery bypass graft.

Vipul has been commenced on:
– Isosorbide mononitrate 30mg daily
– Metoprolol 100mg twice a day
– Clopidogrel 75 mg daily
– Simvastatin 40 mg at night

You and your mentor are assigned to look after Vipul during your shift on the Coronary Care Unit (CCU). Following his heart attack, Vipul has been very subdued and is scared of what will happen to him and how he will provide for his wife and daughter and ‘pay the bills’. You and your mentor have been asked to write a care plan for Vipul and provide some guidance on lifestyle changes and health promotion.
