A scoping exercise to determine if patients are using the most appropriate primary care health service available to them. 

– Gather data on UCC / GP utilisation from UCC data and streaming information over the period of 1 month 
– Identify reasons why patient’s are / aren’t accessing certain areas of primary health care 
– Develop an information poster / flyer for patient’s to assist them in obtaining the most appropriate healthcare
– In doing so, better management of patients expectations will occur
– Write an essay / report outlining how service delivery can be improved from a better understanding of patient expectations.

This report focuses on Urgent Care Centre. You must write a report on what has been found in the patient survey, what implications it has within the healthcare sector, what can be done to change moving forward etc. 

Whilst this is not a reflective case study, this report needs obtain an element of reflection which would make reference to the client’s practice. The client is an Emergency Care Practitioner, with a Paramedic background. They work in an Urgent Care Centre and treat all patients from children to adults. They see, diagnose, treat, refer and discharge a wide variety of illness and injuries.

See attached for UCC.Data.2015-2017.xlsx. This is a spreadsheet which contains all the data for the Urgent Care Centre for the past 2 years; showing total attendances, discharges, referrals. This data can be used if it is felt that it is needed.

1. Evidence of the identification and negotiation of work based learning objectives and solutions which address an initiative for practice development.

2. Evidence of integration of prior and contemporary knowledge to critically discuss relevant issues. 

3. Evidence of offering solutions based on best evidence/best practice for a given area of health care provision.

4. Evidence of critical appraisal of the contribution of their professional development in the delivery of effective care.

5. Evidence of critical reflection on and evaluation of their own performance in terms of knowledge and skills developments
