PH 114: Health Behavior Behavior Change Project: Final Detailed Plan

The final paper is a scholarly written paper with your professor as the audience. So you will need to take command of the words learned in class and apply them to your behavior change project. 

 

 

Submit no more than 5 double-spaced pages (not including the title page or appendices) and 11 font size.  Your writing will need to be concise and specific.

 

Points will be deducted from this content total for format, including grammatical and typographical errors (.5 points per error) and if your paper includes 5 or more errors on the first page, it will be returned to you for resubmission and 10 points will be deducted.   For late submissions, 5 points will be deducted per day the assignment is late. The total points, including the content above, is 45 points. 

 

 

Use the following headings and content for your final paper: The final paper needs to be scholarly written, including the use of words learned from each of the chapters. Cover each section below with specific details.

Review of the literature (8 points)

o  
Conduct a literature review related to the behavior you wish to change, including statistics.

Include a minimum of five references from peer reviewed journal articles.

o  
References must be in APA format in the text and reference list.

Introduction (3 points)

o  
In a 2-3 sentences, describe what your plan attempted.

o  
State the outcome.

 

·       
Goal Statement (3 points)

o  
State the specific, measureable, achievable, relevant and time-bound* goal you selected.

* make sure there is a starting and ending date included

 

·       
Plan To Change (10 points)

o  
Name at least five techniques from Chapters 5-7 or class lectures, and describe how you used each technique.

o  
Name at least five obstacles, barriers or troublesome antecedents that you encountered and how you controlled them.

 

·       
Theoretical Application  (8 points)

o  
Apply either the HBM, TPB, or TTM, to your selected behavior. Be specific in applying each of the constructs to your selected behavior change project and how you will use theory to improve your future health behavior change efforts.

Results (3 points)

o  
Did you accomplish your goal?  Explain. What went well and what didn’t work well?

Conclusions (3)

o  
 Write at least three statements on what you learned.

My Plan for the Future (3)

o  
What will you do to maintain your gains? Include at least three actions.  If unsuccessful, include a second plan for change.

Appendix (4)

o  
 Include a table of your behavior metrics and a graph of the results.  The graph will link your goal to your results. The table shows what you are graphing.  Show the frequency, duration and intensity of your results in your graph(s).

EXAMPLES for Theoretical Application:

You could use Stages of Change (Transtheoretical) model to identify where you were on the continuum of change and if the activities you chose were appropriate to a person in that particular stage of change.  What stage are you in now?  How can you prevent yourself from sliding backwards? What can you do to help yourself continue along the stages of change?

 

You could use the Health Belief Model to illustrate what your cues to action have been and what perceived benefits you have.  What is your perceived susceptibility and seriousness?  Describe your self-efficacy towards your behavior.
