Purpose of Report

Ludmilla is a 46-year-old woman who is depressed after his husband, John, is ended a relationship with her. She also has grief after his father’s loss and the unstable health condition of his mother. This report will aim to ascertain Ludmilla’s relationship style based on her relationship background and how she presents herself. It will also look at how the loss of his father has affected her life.

GENERAL OBSERVATIONS

I met Ludmilla on the streets of New York City. She was well dressed, but she was walking alone. She looked stressed and moody. She was of average height, and she was underweight. The speed of her speech was average, but at times she looked agitated.

INFORMANT

Ludmilla participated sufficiently in the interview. She looked open and ready to share some information. She needed help. I can judge her accuracy in giving information as being good. She is a reliable informant. We walked down to a hotel and gave her the questionnaire our teacher had provided us. She filled it as we shared a cup of coffee.

FAMILY HISTORY

Ludmilla is the only daughter of Marina and Scott James, who broke up when she was 15 years old. Ludmilla does not know the names of her maternal and paternal grandparents. She did not see her paternal grandparents since they died a long time ago; it’s a long time since she met her maternal grandparents because the relationship between her grandparents and her mother is complicated. Marina brought up Ludmilla but currently resides in a rented house. Her parents had a history of depression. Marina was diagnosed with major depression when Ludmilla was eight years. Ludmilla suspects that her father had depression, although he was not diagnosed. Ludmilla has been lonely his entire life. She used to meet his father only on weekends, but she stopped after getting a nursing Job. Sadly, his father died the other day.

Subsequently, her loneliness was exacerbated by her mother when she locked her in a room. Ludmilla lives alone and has maintained a good relationship with his father and her mother, Marina. She spends most of her time with his father at the working place. Ludmilla felt awkward since she did not have much in common with her father. Since her mother had depression, most of the time, Ludmilla was self-dependent. Ludmilla was not close to his mother’s parents, and her mother was not close too. On the other side, she did not have time to spend with his father’s parents since they died long ago.

RELATIONSHIP AND SOCIAL HISTORY

Ludmilla had their first relationship when she was 14 years old. The first one lasted for six months. The second took three years. My third boyfriend was Bill, staying for two years. She met Stephen and got married seven years ago.

The relationship between Stephen and Ludmila took five years before planning to marry each other. Olivia saw his husband as a friendliness, open-minded, and extraordinary gentleman. They have only one kid, Alex. They broke up 3 years ago. Ludmilla did not give clear reasons for their break up, and I opted to leave them.

Ludmilla does not have a lot of friends, just like her mother. The friends she had when she was in university became strangers. Ludmilla has been making efforts to make new friends, but it has failed.

HISTORY OF PRESENT ILLNESS

Ludmilla was diagnosed with depression when she was 26 years old. She does not know when she started going to therapy. She only remembers the current medications. She started treatment this year in January at Charlotte health center. They have been giving her cognitive behavior therapy. They have been giving her Sertraline medications, and they have been doing good in her life. The loss of his father exacerbated her depression. She feels lonely, and she even lacks the energy to cope with daily activities.

Ludmilla has the ability to complete tasks on time. She showed high concentration during the interview, although she stated she found it difficult to focus on his nursing. She plans to quit her nursing job due to her back pain affecting her performance.

Their sleep pattern of Ludmilla is questioned. She manages to sleep only 4 hours a day. Ludmilla has no problem with eating.

MENTAL STATUS

The speed of Ludmilla’s speech is within the standard address. She showed high cooperation during the interview, and she is reliable in giving out information. Her thoughts are logical, and she reasons well. Through the questions I asked her, her judgment and insight could have been better.

SUMMARY

Ludmilla has a background history of psychological illness. Her mother suffers from depression issues, and her father has undiagnosed depression. Marina had relationship issues with his parents, which affected Ludmilla’s relationship with his grandparents. Ludmilla has never had a long-term relationship, even with her friends, exacerbating her depression.
