Outcomes assessed in this Assignment: NU416-5: Implement evidence-based interventions for managing the chronic care of patients to promote health and quality of life across the lifespan. Adherence to a medication regimen is a key factor in promoting health and quality of life in those with chronic illness. Non-adherence can be attributed to a multitude of factors including cost, complexity of the medication regimen, and actual or perceived side effects (Ferdinand et al., 2017). Nurses in both acute care and community or public health settings play an integral role in increasing medication regimen adherence to those with chronic illness (Shin & Haberman, 2016). For this two-part assignment, you will create an evidence-based plan to increase quality of life and promote health through improved medication adherence. Part 1: For this plan, complete the following: 1. Survey your place of employment or community to identify a common chronic illness. For example, if you work in a public health setting, do the people of your community have an increased incidence of heart failure? 2. Using current and relevant nursing evidence as a guide, describe a common medication regimen for the chronic illness identified above. 3. Develop a plan that includes at least four nursing interventions that will increase adherence to the medication regimen. Please support each of these interventions with evidence-based nursing sources. This plan should be no more than two pages in length. When creating this plan: · Considers the client’s level of understanding. · Do not use medical terms the client and family might not understand. · Individualize the plan to your client’s (individual or people group) needs and style of communication. Part 2: Identify an individual or group of people that could benefit from this plan. Share this plan with them and briefly describe their receptiveness to it.
